She is survived by her son, William Johnson, Sr.; sister, Loretta (Ralph) Sheldon; brother David Merkley; grandchildren, William Johnson, Jr., Matthew Johnson, Layla Johnson; great grandchildren, Brooke Johnson, Amiah Johnson; several nieces and nephews.
Hazel was predeceased by her long time companion, Larry Jones; brother, Carl Merkley; sister, Patricia Kramer.
Services and interment will be held at the convenience of the family. Private interment Oak Ridge Cemetery, Lima.
